CANCELLATION POLICY REMINDER: Our office requires a full 24-hour cancellation of any/all appointments. Appointment(s) missed or cancelled outside of this window will be charged in full.

REMINDER FOR PRESCRIPTIONS:
Please request a refill for prescriptions from the pharmacy directly. They will fax the refill authorization to our office from there. Without that authorization request, we cannot call in refills. Please give yourself at least 1 week before you run out to call in a refill to allow both the pharmacy and our office time to fill.

The patient portal is designed for simple yes or no questions, plan clarity and support as indicated by your practitioner. As of 2024 an active membership is required to direcrt message your practitioner in the portal.

If your portal question requires more time, it is subject to billing for an E-Consult at the discretion of your Practitioner.

As always, Please call 911 if you are having a medical emergency or go to your nearest hospital.
